There are three types of NDIS therapy options:

Each of the three options have specific documentation to be completed. There must be a form completed per child.

1. Meeting - Please complete Attachment A - this must clearly state the date and frequency of meetings, duration and signature of parent/carer giving consent.
2. Observation - Please complete Attachment B - this must clearly state the date and frequency of observations, duration and signature of parent/carer giving consent.
3. Session - Please complete a licencing agreement (available on request) and complete Attachment C - this must clearly state the date and frequency of sessions, duration and signature of parent/carer giving consent. You will also be asked to provide a copy of any session notes after each session as stated in the licencing agreement.

Licencing Agreement:

You must complete a licencing agreement if you are wanting to hold therapy sessions at Ballarat Specialist School (available on request) and provide a copy of:

1. Valid working with children’s card (for all practitioners attending the school)
2. Valid Liability insurance – Certificate of Currency.
3. Signed Information sharing deed Attachment D (this can be within the licencing agreement or submitted later and must be for each therapist)

Licencing agreements are for the calendar year. New agreements will be available towards the end of Term 3 of the preceding year. After the commencement of the school year agreements will be for the remainder of the year only.

Sessions will not begin until all documentation has been received.
