NDIS Therapy at Ballarat Specialist School

Thank-you for your interest in providing NDIS therapy to students at Ballarat Specialist School in 2025.
On this page you will find information on how to apply for and book NDIS in school therapy for students at both the Gillies Street Campus and Norman Street Campus.
Before you can undertake NDIS in school therapy you must enter into a licencing agreement Attachment A with School Council and provide the necessary documentation. All previous School council NDIS Funded Therapy Licences will conclude at the end of 2024. If you would like to continue to provide in school therapy, a new one must be completed for 2025.
The licence must be approved by School Council. The Council only meet twice a term, as a result please expect a 4-6 week wait to commence. If you complete a licence before the 27th of November, you will avoid any wait time in 2025.
You are also asked to provide or undertake the following:-
- Provide a copy of your Certificate of Currency (liability insurance)
- Submit your request online per student (adjacent)
- Complete a Request Form and Parent Consent Form per student Attachment B
- Complete an Information Sharing Deed per student Attachment C which must be signed by the school Principal
- Complete an OHS induction and checklist one time for 2025 Attachment D
- Supply a copy of your Working with Children’s Card
- Familiarise yourself with the following documentation/policies:
- Privacy Policy (listed on the “about us” tab on this website)
- Child Safety – Code of Conduct (listed on the “about us” tab on this website)
- Visitor Policy (listed on the “about us” tab on this website)
- OHS Handbook & Policy (policy forms part of the OHS Handbook) Attachment E
- Agree to the Observation Guidelines one time for 2025 Attachment F
- Pay the licencing fee for room hire (if applicable) as stated in the licence per booking
- On conclusion of therapy, provide session notes as agreed in the information sharing deed.
It is recommended you submit a soft application (adjacent) to get your request started. This will go through a number of checks before a decision is made by the Principal. You will be notified via email with the outcome of your request. A semi populated Request Form and Parent Consent Form will be sent if approved. You will be required to check data, finalise this document and return to the request email address at
While every effort is made to support in school therapy, there are times where it can not be facilitated which include:
- individual circumstances of the student
- student access to the curriculum
- Child Safe Standards and duty of care including the ability of school staff to supervise the student while the NDIS funded therapy is being provided
- practical and administrative capacity
- anti-discrimination obligations.
If your request is not approved the parent/carer will receive written notification via email. You will be copied into this communication.
